Job Description:
Under the direction of the Assertive Community Treatment (ACT)
Team Leader, this position will be responsible for coordinating and
implementing psychiatric services for all team consumers, and
provision of medical management services for up to 40 consumers and
case management services for up to 5 consumers. Assertive community
based provision of services will require a flexible schedule and
will include on-call responsibilities. Qualifications: Must be a RN
and a minimum of 2 years of experience in a mental health setting.
Must possess the ability to work independently and as a part of a
team. General Responsibilities: 1. Assist consumers in identifying
and accessing medical needs through regular assessment, and
coordinating and assessing necessary medical services. 2.
Demonstrate a Rehabilitation/Recovery value system toward service
delivery by providing consumer-driven strength based approach to
medical services, case management and crisis services. 3.
Coordinate service delivery by other team members in relation to
medical services. 4. Coordinate service delivery, both internal and
external, in relation to medical services. 5. Complete initial
nursing evaluations, including medical history summary and yearly
updates on each consumer. 6. Provide medication administration,
monitoring, education, and recording as needed. 7. Work closely
with psychiatrist(s) to follow through with MD orders. 8. Assess
consumers for symptoms and side effects of medication and provide
education, management, and documentation as needed. 9. Educate and
train staff, family members, and community providers in relation to
all areas of medical treatment and its relationship to psychiatric
treatment. 10. Maintain Pharmaceutical supplies, and other
treatment supplies as required for assessment and treatment. 11.
Participate in service delivery for all the consumers on the team.
12. Participate in service plan meetings, treatment meetings with
other providers, and team clinics, as indicated by team leader.
Follow through with clinical decisions as determined utilizing a
team approach to problem solving. 13. Act as primary case manager
when needed by assisting consumers in obtaining and maintaining
basic living needs and skills related to housing, food, psychiatric
and medical care, transportation, socialization, education, and
employment in a manner that is clinically appropriate. 14. Attend
and participate in all meetings, supervisions, and trainings in a
professional manner. 15. Act as a liaison with family, service
providers, and community supports and services, as needed and
appropriate 16. Maintain professionalism as per corporate policy
during all aspects of service delivery. 17. Fulfill on-call duties
as assigned. 18. Complete all paperwork in an accurate and timely
manner. 19. Assure necessary documentation is completed in a timely
manner to meet CBHNP and program requirements. 20. Complete all
