Assistance Program Job Summary: The Community Health Worker is
responsible for identifying low-income individuals and families and
assist them with navigating the health care system, including
arranging for assistance with obtaining health benefits, connecting
to a primary care provider or free medical service, and/or
accessing needed mental health, dental, and vision care. The
position will educate individuals on the appropriate utilization of
the health care system to reduce emergency room visits. This
position requires extensive field work requiring the ability to
work some evenings/weekends and regular travel throughout the
Hamilton service area. Essential duties and Responsibilities:
Establish and maintain trusting working relationships with patients
and their families while providing general support and
encouragement.Identify and connect uninsured or underinsured
patients to benefit coordinators or other insurance enrollment
specialists.Conduct intake interviews with patients and refer
patients into appropriate social programs.Follow-up with patients
via phone calls and visits to other settings where patients can be
found.Follow-up with patients should be continuous from initial
contact through closure.Assist patients in establishing a
relationship with a designated primary care provider (PCP), dental,
vision, mental health, and supportive service partners.Distribute
culturally appropriate health information.Help patients set and
attend appointments.Help patients connect with transportation
resources. Transporting patients is strictly prohibited.Help
patients navigate systems to obtain resources and or connect with
services, internally and externally. Minimum
Education/Certifications: High School diploma or equivalent with
comparable work experience and strong attention to detail.
Completion of a Community Health Worker formal training program,
preferred. Knowledgeable of some medical terminology and bi-lingual
speaking preferred.Minimum Work Experience: Minimum of one year of
experience working in a community-based setting performing care
coordination /care management in a medical setting, preferred, but
not required if other relevant experience can be demonstrated.
Ability to clearly communicate issues to all levels of
management.Other Requirements: Reliable transportation to travel
throughout the service area. Able to work some evenings and
