Job SummaryWorks within the community providing outreach to patients with complex medical and social needs. Acts as a resource to assist patients in navigating the health care system.
Job Duties

• Assists in the identification and outreach of medically complex patients.
• Collaborates with the multidisciplinary team in the development and implementation of the care plan.
• Engages patients by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.
• Completes Vital Signs
• Identifies gaps in care and connects members with resources
• Facilitates telemedicine visits with members and providers
• Performs disease specific education
• Conducts safety evaluations in patients home
• Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
• Provides reinforcement of an established treatment plan.
• Builds relationships within the community in order to identify resources that are available to the patients i.e.: DME vendors, Home Health Agencies, Area Aging Office etc.Work is typically performed in a community environment.   Position Details
  • Hours: 8 AM to 4:30 PM with 1 later day per week from 9:30 am to 6 pm
  • Travel: Yes, this is a community-based role. Company car will be provided.
