Care Schedule: Monday–Friday, 8:00 am – 4:30 pm Shift Type: Days |
No weekends | No on-call required Overview We are seeking
experienced and compassionate Registered Nurses (RNs) to join our
hospice care team. The RN will provide high-quality clinical care
to patients receiving end-of-life services, ensuring dignity,
comfort, and support for both patients and their families.
Candidates must be comfortable working independently, managing a
daily caseload, and delivering comprehensive nursing assessments
and interventions across a variety of care settings.
Responsibilities - Perform comprehensive nursing assessments
including admissions, recertifications, routine, PRN, and death
visits. - Develop, implement, and evaluate individualized care
plans in collaboration with the interdisciplinary team. - Ensure
quality and compliance through accurate and timely documentation in
the EMR system (HomeCare Homebase experience strongly preferred). -
Maintain productivity benchmarks of 5–7 visits per day while
delivering compassionate, patient-centered care. - Coordinate with
physicians, social workers, and other healthcare professionals to
facilitate care transitions and appropriate levels of service. -
Identify and address social determinants of health that impact
patient care (e.g., housing, transportation, or community
resources). - Promote health education, preventive care, and
emotional support for patients and families. - Advocate for patient
rights while maintaining confidentiality and ethical standards. -
Participate in interdisciplinary team meetings and contribute to
continuous quality improvement initiatives. Qualifications - Active
RN license in good standing (state-specific). - Current BLS/CPR
certification required. - Valid driver’s license and auto
insurance. - Hospice experience required; HomeCare Homebase (HCHB)
proficiency strongly preferred. - Associate’s or Bachelor’s degree
in Nursing (BSN preferred). - Strong clinical judgment and
critical-thinking skills in complex situations. - Exceptional
communication and organizational abilities. - Case Management
certification preferred, but not required. Key Competencies -
