care. Responsibilities: - Provide high-quality care to residents,
including assistance with activities of daily living (ADLs), such
as bathing, dressing, and grooming - Monitor vital signs, report
changes to nursing staff, and implement care plans as directed -
Assist with medication administration, including tracking and
reporting medication usage - Develop and maintain positive
relationships with residents, families, and coworkers - Participate
in care conferences and contribute to the development of
individualized care plans - Maintain accurate and detailed records
of resident care and services - Utilize effective communication
skills to ensure seamless care transitions and collaboration with
healthcare team members Requirements: - Current CNA certification
in the state of Pennsylvania - High school diploma or equivalent
required - Ability to lift, transfer, and reposition patients with
