Job Description:
Job Description Job Description We are looking for a
compassionate RN to join our growing team! The right candidate has
a can-do attitude, a friendly demeanor, excellent interpersonal
skills, and the ability to provide medical support to our clients
in their homes. Responsibilities Communication skills to work with
nursing staff for implementation of care plan Oversees care plan
meeting with resident and family Assures that care plans are keep
current and reflective Verifies that assessments transmitted to
database with necessary corrections Reviews of the MDS for accuracy
for any trends that need education Monitors documentation of
Medicare and managed care resident and educate as needed Monitoring
of QM with emphasis on those impacting five-star rating Experience
in CMI for best outcomes and guidelines for documentation required
Knowledge of restorative programs and implementation Participation
in Quality Improvement program in the facility Have 2-4 years work
experience in MDS and long term care Point Click Care experience
preferred Qualifications RN degree from an accredited college
(minimum ASN) Currently registered in the state Have AANAC
certification Must have current CPR certification We are an equal
