Job Description:
ChristianaCare, a leading non-profit health system in Delaware,
is seeking a dedicated RN Case Manager to join our team and
contribute to our mission of delivering exceptional healthcare
services across our specialties. In this vital role, the RN Case
Manager will oversee patient case management, ensuring seamless
transitions, comprehensive care plans, and effective utilization of
resources to promote optimal health outcomes. Assess new patients
by evaluating their needs and eligibility for services Develop,
implement, and review care plans in collaboration with other health
care professionals Ensure resource utilization effectively matches
patient care requirements with available services Monitor patient
progress and adjust care plans as necessary Advocate for patients'
needs and act as a liaison between patients, their families, and
healthcare providers Educate patients and their families about
health management and available services Document all patient
interactions and decisions related to case management Promote
continuity of care by coordinating with outside providers and
managing transitions between care settings Strong knowledge of
disease management and treatment Excellent communication and
interpersonal skills Ability to make clinical assessments and
decisions Proficiency in healthcare software and databases Strong
organizational and time-management skills Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current Registered
Nurse (RN) license in the state of Delaware Certification in Case
Management (CCM) preferred
