These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members.Join Landmark to be part of a growing company full of purpose-driven, action-oriented, and compassionate team members working to dramatically transform healthcare for our communities.The Community Outreach Representative -is responsible for increasing patient engagement through telephonic outreach to Landmark members to schedule an in-home clinical visit by a Landmark provider.

  • Utilize tried and true telephonic methods of patient outreach and engagement strategies to schedule visits for enrollment into the Landmark Health program.
  • Utilize excellent telephonic sales skills to effectively communicate Landmark services and be able to overcome members objections. -
  • Uses critical thinking skills to create telephonic outreach strategies that are designed to maximize patient awareness and registration into Landmark's program. -
  • Leverage reporting and dashboards to prioritize telephonic outreach to members
  • In coordination with Local Outreach Team, support telephonic outreach to non-engaged and engaged patients.
  • Successfully meet and/or exceed engagement monthly targets
  • Reports directly to the Manager, Community Outreach and EngagementThis position is full-time (40 hours/week) Monday to Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 08:30am - 05:00pm EST. It may be necessary, given the business need, to work occasional overtime. Our office is located at 2401 Park Drive, Harrisburg, 3114.*All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.Primary Responsibilities:

      • High School Diploma / GED (or higher)
      • Experience utilizing Microsoft Word, Microsoft Excel and Microsoft PowerPoint

          • Bachelor's Degree -(or higher)
          • 2+ years of related experience in a sales, direct marketing or customer service capacity
