Job Description:

*Homeland Hospice is looking for a compassionate and caring Social Worker, driven by a desire to provide support to terminal patients and their families. This candidate should also be a team player, willing to work in coordination with the whole care team, providing care for the patient's whole being - body, mind, and spirit.*

* *This is a Full-time, Monday through Friday, Daytime position.*
* *This position provides advocacy for terminal patients and their families located in the Greater Harrisburg region, with paid mileage.*

_Professional position responsible for the provision of a wide range of psychosocial services to terminally ill patients and their families, including completing the comprehensive psychosocial assessment and regularly assessing the patient and family psychosocial needs on an ongoing basis. The social worker also provides the bereavement risk assessment of the family and counseling relative to anticipatory grief issues. Upon completion of the assessment, a psychosocial plan of care is developed with the Interdisciplinary Team (IDT) and the patient/family and it is the role of the social worker to implement and continually revise and reassess the plan relative to the psychosocial and potential bereavement issues of the patient and family. The services provided may include: supportive counseling, problem solving, community referrals, pre-bereavement counseling and support._

*Essential Duties and Responsibilities* include the following.

_Social Work Services_

* Conduct initial comprehensive psychosocial and bereavement risk assessment of patient/family/caregiver in a thorough, accurate, and compassionate fashion within five (5) calendar days after the patient's election of Hospice care.
* Function as a support person for the patient/family/caregiver on an ongoing basis.
* Function as an expert and facilitator at obtaining and making referrals to community resources to meet the needs of patients and families and obtaining financial assistance when indicated.
* Coordinates resources for respite care needs.
* Assist family/caregiver in placement of patient in a facility for basic care, respite or inpatient when appropriate as indicated.
* Provide psychosocial and anticipatory grief counseling to patients, families, caregivers and significant others relative to the impact and implications of the terminal disease and patient care.
* Attend and actively participate in IDT meetings, staff meetings and in-services, and contribute positively to patient care planning.
* As directed and requested by the Director, Assistant Director of Social Services, Assistant Director of Clinical Services, Assistant Director of Support Services, Spiritual Counselor or Bereavement Counselor, conduct bereavement visits or contacts and/or support groups as appropriate.
* Works in concert with other Hospice IDT members.
* Provide and maintain a safe environment for the patient.
* Assist patient/family/caregiver and other team members in providing continuity of care.
* Maintain caseload more than 15 patients at any given time or as requested.

_Resource Utilization_

* Coordinates travel time for visits and manages a caseload to deliver care in a cost-effective manner that demonstrates an understanding of Hospice reimbursement methodologies.
* Assure proper maintenance of records per agency guidelines.
* Participates in evening/weekend on-call as required, conducting on-call services in a clinically competent and responsive manner.
* Maintains regular attendance
* Functions independently but seeks and accepts guidance from other members of the IDT and members of the community, as deemed necessary and appropriate.
* Complies with local, state and federal laws, Medicare Hospice Conditions of Participation and agency policies and procedures.
* Conducts all business activities in a professional and ethical manner.

_Customer Service_

* Demonstrates the highest degree of customer awareness by seeking out opportunities to identify and meet the needs of internal and external customers.
* Treats each person as an individual with respect and dignity.
* Is receptive to questions and criticism, and willing to offer assistance.
* Demonstrates genuine concern and empathy.
* Consistently performs all of the above in a friendly, courteous manner.
* Maintains accountability for patient satisfaction throughout the episode of care.

_Quality Assurance/Performance Improvement (QAPI)_

* Participates in the performance improvement process by finding opportunities for continuous monitoring and identification of patient outcomes, intra/interdepartmental and environmental concerns.
* Displays a willingness to utilize problem solving techniques to insure appropriate interventions and resolutions.
* Participates in clinical record review.
* Assists and serves on committees and projects as requested.

_Secondary Duties_

* Performs speaking engagements on Hospice and provides in-service education to contractors, referral sources and other appropriate groups, as required.
* Performs other duties as assigned.

*Requirements: *

_Education and Experience_

* Has a Master of Social Work (MSW) degree from a social work program accredited by the Council on Social Work Education; and/or Bachelor Degree in human service field and/or minimum 1 year of social work experience in a healthcare setting.

_Licensure, Registration, and/or Certification_

* Cardio Pulmonary Resuscitation (CPR) certified.
* Must be a licensed driver with an automobile in good working condition and have automobile insurance in accordance with Pennsylvania laws.
* Must have reliable internet access in their place of residence.

Pay: $25.00 - $33.00 per hour
