We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team and provide compassionate medical care to individuals in our residential and community programs. If you are a compassionate LPN looking to make a difference, we'd love to hear from you!*Key Responsibilities:*--- Provide direct nursing care, including medication administration, wound care, and monitoring vital signs.--- Conduct health assessments and report concerns to healthcare providers.--- Assist with medication management, education, and compliance for individuals in our programs.--- Maintain accurate medical records in compliance with state and organizational policies.--- Educate and support staff and caregivers on health-related concerns and best practices.--- Collaborate with case managers, therapists, and community resources to coordinate care.--- Advocate for clients' health needs and ensure a safe, supportive environment. Qualifications:--- Active LPN License in Pennsylvania (required)--- Driver's License (required)--- Experience in mental health, intellectual disabilities, or community-based care (preferred)--- Strong communication and interpersonal skills--- Ability to work independently and as part of a team--- CPR & First Aid Certification (or willingness to obtain)--- Passion for serving vulnerable populationsJob Type: Full-timePay: $52,520.00 per year_*Minimum requirements*__: HS diploma/GED, current PA LPN license, and two (2) years of professional experience; ability to complete physical and PPD tests. Must be at least 21, possess a valid driver's license, insurable driving record, and reliable transportation. Current physical and Mantoux (TB) test required. PA State Police background check, Act 33, and/or FBI clearances.
